Here’s some key info for you to know. Please read this before filling out the cancellation form.
Required Notice – Cancel Anytime Members must submit their request 30 days before the desired termination date. Annual Members who wish to terminate their agreement early must submit their request 30 days before their desired termination date and pay the termination fee and the time of termination.
Account Balances and Terminations—Members with outstanding account balances may not terminate their membership until their balances are paid.
Other termination types - If you have a medical issue that prevents you from utilizing your membership, you may be allowed to terminate without penalty by providing a note from your doctor detailing the medical necessity of the termination. If you move and your new home is more than ten miles from an Elevate facility, you may terminate without penalty.
